

We support persons with disabilities (PWDs) facing life-limiting and life-threatening conditions and their carers by providing comprehensive community care. It aims to enhance the quality of life and dignity of PWDs during the final stage of their journey.
Persons with disabilities need specialised care when they develop serious illnesses
As people age, how individuals respond to serious illness is deeply personal and can vary widely. However, persons with disabilities (PWDs) often face even greater challenges, such as increased risks of complications from illnesses, premature ageing, and psychological stress. With over 530,000 PWDs in Hong Kong, approximately 70% of whom are aged 50 or above, there is a pressing need for additional specialised care.
The Hong Kong Jockey Club launched the Jockey Club End-of-Life Community Care Project: Unison (JCECC: Unison) to support PWDs with life-limiting and life-threatening conditions. The project brings together healthcare and social care professionals to provide comprehensive care in addressing the physical, psychological and practical needs of PWDs and their carers.
JCECC: Unison provides customised care based on the needs of each individual. This includes physical symptoms management, caregiving skills training, and rehabilitation equipment access. Practical support extends to home modifications for safety and ease, alongside guidance through available community resources.

The project also pays equal attention to emotional and psychological wellbeing. Trained professionals provide emotional support and counselling sessions to improve family relationship. They guide families through sensitive decisions from care planning to funeral arrangements. One of the keys to the project is a commitment to promote autonomy in end-of-life decisions, ensuring PWDs’ voices remain central to their care. Bereavement counselling is also offered to families in supporting continuously after their loss.
Understanding the crucial role of carers, the project eases their burden by organising volunteer teams to create a support network for those facing emotional strain and practical challenges.
Beyond direct care, the project works to enhance public awareness through conducting activities and community workshops to encourage open, compassionate conversations about end-of-life issues, fostering greater understanding and empathy within society.
