Jockey Club Community eHealth Care Project

Primary healthcareDigital & technology enhancement

We help the elderly take charge of their own well-being through community support and senior-friendly digital health technology. We have empowered over 82,000 elderly to monitor their health measurements regularly.

75% of people in Hong Kong aged 65 or over are suffering from one or more chronic diseases

The ageing population in Hong Kong is growing year on year, as is the number of senior citizens affected by chronic medical conditions such as hypertension and diabetes. As such, the promotion and maintenance of self-health management and a healthy lifestyle among the elderly has become a pressing issue that we need to tackle together as a society.

We launched the project as the first elderly care project in Hong Kong to integrate digital health management, community care, and professional support. To promote chronic disease management and healthy ageing, leverage senior-friendly technology to empower self-management capability and improve health literacy of older people. The data we derive from the project will be utilised for broader analytics and greater synergy between electronic health records to enhance existing healthcare services.
